Many insurance companies are using software programs, like Colossus, for claims settlement instead of leaving it up to the adjusters to figure out. The claims adjusters glance through the medical and chiropractic records, looking for key words and phrases called "value drivers." If these value drivers are not documented, the value of the claim becomes less. If the value drivers are properly documented, the value of the case increases. These value drivers include range of motion, ICD9 codes, CPT codes, injuries, muscle spasm, treatment, impairment ratings, duties under duress and loss of enjoyment and prognosis. In order to properly document personal injury cases, the examinations must be thorough, re-examinations must be done at regular intervals and daily SOAP notes must include the items that Colossus and other programs look for, when they exist.
