"When I signed on for twelve hours of "Documentation," I was prepared for a painful snooze fest of mundane information that would be retained and absorbed in a similar fashion as one might expect of a Tabasco enema.  Believe me, I, like most of my colleagues, need a record overhaul, but doing something about it is a task not relished or performed in a timely manner.  We all dread the possibility of audits but do little to avoid them, largely because we have no idea how and what to change, and who to ask.

I can't tell you how many transient or transferred patients that I have taken care of who's former DC should be flogged due to his inability to actually examine the patient AND put those results on paper. There are never any really great examinations being done out there and the few that there are rely on orthopedic test that, I now know, hold very little water in a courtroom. "Dear Dr. Friedman: I attended the March 18, 2004 seminar in Seattle. Recognizing that you have well established your fine reputation with well known attorneys, i.e. Scott Blair, I feel a bit pretentious adding my praises. That having been said, I thought the seminar was superb. Unfortunately, I came away from the seminar with mixed feelings. On one hand, it was gratifying to hear you "cut to the chase" and describe the basic problem that has kept the chiropractic community always behind the insurance "8 ball"; namely, records. More specifically, "communication". I have spoken at several meetings and seminars for chiropractors and have repeatedly emphasized the joys of clear, clean recordkeeping. This leads me to the "other hand". Namely, that recordkeeping leads to the need for precise measurement. It has been one of my greatest frustrations to read "chiropractic reports" that go to great length discussing such things as the "fibrosis of repair" and "subluxation" but fail to give any clear indication as to the status of the patient. Not enough of your brothers and sisters have gotten the message and I continue to ask myself: 'How in God's Green Earth am I going to prove my client's injuries in absence of accurate measurement and reporting?'
